Courage Kenny Rehabilitation Institute provides rehabilitation services for people with short- and long-term conditions, injuries and disabilities. The Institute includes 3 unique inpatient facilities providing inpatient level rehabilitation. The Courage Kenny Transitional Rehabilitation Program is a 48 bed facility licensed as a SNF and located on the Golden Valley campus. Its unique licensure allows clients to continue in the rehabilitation journey longer than their hospital inpatient rehabilitation stay with the goal of return to a community living setting. As a provider in this setting you have the very unique opportunity to provide medical care within this neuro-rehab setting. You are part of a highly specialized team including physiatry, nursing, all therapy disciplines (PT, OT, ST), psychology, neuro-psychology, dietician, social services, and therapeutic recreation. In addition, psychiatry and pain specialty consults are available on site.

Provides overall care and rehabilitation plan for patients within the Transitional Rehab Program (TRP)
Functions appropriately as part of care delivery team through efficient use of resources and skills, both their own and those of other provider and staff
Actively participates in multidisciplinary team approach to case management
Quality improvement utilizes quality improvement tools to develop processes that promote continuous improvement in delivery of patient care, patient outcomes or workflows
This position requires a Medical Doctorate degree (M.D.), Doctor of Osteopathy (D.O.) or foreign equivalent from an approved school of medicine
