Created in 2011, the DSSP 12 month fellowship program strives to provide comprehensive interventional spine, sports, and musculoskeletal education in a private practice setting that stresses ethical and evidence-based medicine. We have four board-certified PM&R physicians who practice out of two offices in Phoenix and Mesa which include fluoroscopic procedure suites and on-site physical therapy.
We select one to two fellows each year who work directly under the supervision of a DSSP physician. A typical week consists of a blend of both outpatient clinic and fluoroscopic spine and joint procedures. Outpatient clinic includes evaluation and management of spine (70%) and musculoskeletal (30%) pain, comprehensive neuromuscular examination, and ordering and diagnostic image interpretation of pain films, MRI and CT. We see patients across the lifespan with acute and chronic pain however we do not utilize chronic opiate management. Outpatient clinic also include electrodiagnostic studies, ultrasound guided injections, PRP, and prolotherapy.
The fellow also presents articles at our monthly journal club and delivers a monthly lecture to the practice on a Spine or Musculoskeletal Medicine topic. We also have monthly physical exams and ultrasound guided injection workshops. Teaching is also provided on coding and billing, business skills, and practice management.
Requirements for application to the DSSP fellowship include the following:
Completion of an ACGME accredited residency program in PM&R
ABPMR board-certified or eligible
Three letters of recommendation
Applications should be emailed to Kathryn Meloy, DSSP Practice Administrator at firstname.lastname@example.org. Applications will be accepted from April until September 2018. After completed application materials are received, we will review the materials and contact selected residents for an interview. The interview will take place over two days with time spent with physicians and fellows in the clinic and procedure suit in both offices. The fellowship start date is July 1, 2019 however the start date is flexible pending approval of the fellowship director.

Cervical, thoracic, and lumbar epidural injections (interlaminar, transforminal, caudal)
Cervical, thoracic, and lumbar selective nerve root injections
Cervical, thoracic, and lumbar facet and medial branch block injections
Cervical, thoracic, and lumbar radiofrequency ablation
